This avoids creating ImmutableCollections.ListItr when performing
InputMonitor#populateInputWindowHandle for each window.
The iteration on a pure array is usually 3x faster than collection.

When we create the bitmap for the icon, we specify the size as the same
size as app header icon. Create 2 different bitmaps for resize veil and
the app header icon.

Package name is more commonly used than appid, so we hope to support obtaining permission and appop status based on package name.
